To submit a claim, download and complete an Athletic Accident Claim Form https://www.bcsoccer.net/media/tgrh1pdx/athleticaccidentclaimform20211001.pdf On the bottom right hand corner of Page 1, you'll need the Group Policy Number: CAS665228-05 and the coach's signature. Follow the instructions to start a claim.
